General Information

Client Profiles

A client profile – consultation & medical history packet – will be required at initial visit, prior to treatment. It is imperative that you disclose any medical condition and/or medications you may be taking. Please answer all appropriate questions as accurately as possible. All client profiles are confidential. Scheduling

Availability is limited. In order to assure appointment day and time of your choice, it is recommended that appointments be scheduled at (2) weeks in advance.

Cancellations

Please extend 24 hours notice for canceling appointments.

Day of Services

For first-time visits, please arrive 15 minutes in advance to facilitate timely client profiles. All future appointments are expected to be “On time” as lateness can result in a shortened length of treatment at full price. In order to receive the best treatment experience, please turn off all cell phones during your visit.

Age Requirements

Minors must be accompanied by a guardian on first visit, and must have appropriate waivers. Parental Consent Form is also available HERE

 

Payment

Accepted forms of payment are Cash or Credit Card (Sorry, No Checks) VISA • Mastercard • Discover • Amex

 

Gift Certificates

Gift certificates are available in monetary amounts, are transferable but non-refundable.

Gift certificates can be used towards services and retail products.

Returns and Refunds

Merchandise will be accepted for refund within (30) days of purchase, unopened and with a valid receipt. Services are non-refundable. Questions or problems with a service should be addressed as soon as possible.

Hours of Operation

Wednesday & Thursday: 10:00 a.m. – 8:00 p.m.  •  Friday: 9:00 a.m.– 6:00 p.m..  •  Saturday: 9:00 a.m. – 4:00 p.m.

Hours may vary by season.

All services are provided by appointment only.
