A client profile – consultation & medical history packet – will be required at initial visit, prior to treatment. It is imperative that you disclose any medical condition and/or medications you may be taking. Please answer all appropriate questions as accurately as possible. All client profiles are confidential. As all services are “By Appointment Only”, a credit card is required on file to hold any/all appointments. You will not be charged prior to your appointment and you may choose an alternate payment method at checkout. *(Does not apply to Monthly Maintenance Memberships). Availability is limited. In order to assure appointment day and time of your choice, it is recommended that appointments be scheduled at (1) week in advance. Cancellations, No-Shows & Late Arrivals
24 hours advance notice is required, when cancelling appointments, or a cancellation fee may be charged. This allows other clients the opportunity to schedule an appointment. A client who does not cancel within 24 hours and does not come for appointment is considered a “No-show” and will be charged the full cost of the scheduled session. Depending upon how late you arrive, your appointment may be shortened or cancelled, by therapist – allowing for clients following you. Either way, you will be responsible for the full price of your session.
Day of Services
If you have a fever, cold, illness etc, on the day of your appointment, please CALL BEFORE coming.
For first-time visits, please arrive 10 minutes in advance to facilitate timely client profiles. All future appointments are expected to be “On time” as lateness can result in a shortened length of treatment at full price. In order to receive the best treatment experience, please turn off all cell phones during your visit.
Minors must be accompanied by a guardian on first visit, and must have appropriate waivers. Parental Consent Form is also available HERE

Returns and Refunds
Merchandise will be accepted for refund within (30) days of purchase, unopened and with a valid receipt. Services are non-refundable. Questions or problems with a service should be addressed as soon as possible.
